New Patients

How to register as a New Patient

Any person who is a resident in our catchment area is eligible to register as a patient of the practice.

Please see our catchment area to find out whether your address is within our catchment area before filling out the forms. If you are unsure whether you address is inside our catchment area then please speak to a receptionist on 01322 224550.

Please note: you may know someone who is registered at our practice at an address that is outside of the area, this is because the area has changed several times over the years and their address will have been within our area in the past. We cannot accept new patients from addresses that are outside of the current catchment area.

We require you to complete a GMS1 form and a Registration Form for each adult or child registering at the practice. 

You will also need to hand in a current utility bill and passport for photocopying as proof of your address. Please hand your completed forms to reception in person.

You may choose to see whichever doctor of your choice subject to availability.

In addition please either fill out the Online Registration form (which allows for appointment booking, requesting medication or viewing medical records) OR Contact details & consent form (if you do not wish to register for Vision Online Services). 

Please note: if you fail to attend 3 consecutive appointments you will no longer be able to book appointments online.
